If you need assistance with insurance enrollment, please visit our Medicaid enrollment help page. Please note that no one will be denied services due to an inability to pay. We provide care to the uninsured using a sliding fee payment scale that is based on family size and income.

AVAILABLE: Psychiatry and Counseling appointments. To sign up, stop by or call our health centers that offer these services.

LIMITED: Medical Primary Care and Dental appointments. If you want these services, sign up for our online Integrated Case Management intake process here.
- If you need an appointment: For a same day appointment, call (303) 293-2220, arrive at health center opening, or schedule your appointment online through the patient portal.
If you are interested in joining the patient portal, but do not yet have an account, please call or talk to a front desk staff member at your next visit. They will verify that we have an accurate email address for you and will send an invite for you to join our patient portal. In the patient portal, you can view upcoming appointments and your appointment history, request appointments, view your lab results, request prescription refills, and message with your treatment team.
If you have an appointment:
There is no need to bring anything with you, nor do you need insurance in order to be seen.
However, it is helpful, but not required to bring the following items:
- Photo ID or other form of identification
- List of current medications (or even the bottle)
- Medicaid card, if you have one
- Any paperwork you received, if recently discharged from a hospital
- * For behavioral/mental health appointments: A list of any symptoms you or people close to you have noticed, and how long you have been experiencing these symptoms
- * For a child's appointment: Vaccination/immunization records
Please note:
- We request that bicycles be left outside of the Health Center.
- Patients may only bring service animals into the Health Center. Pets are not allowed.
- We are NOT an Approved Treatment Provider (ATP) through the CO Department of Corrections.
RELEASE OF INFORMATION FORM (ROI)
We are happy to provide you with a copy of your medical record. To request a copy of your medical record, you, or someone you designate must complete the Authorization to Release Health Information Form available below. CCH does not charge a fee for copies of your records that are either delivered to you or to a designated third party. If you have questions, or need instructions on how to request your record by alternate means, please contact Health Information Management at (303) 312-9799.

The mobile medical clinic travels with a Medical Assistant accompanied by a Physician, Physician Assistant, or Nurse Practitioner. On board, HOP providers run two treatment rooms, a small pharmacy, and a lab. New patients are welcome to walk-in for care.

The Health Outreach Program (HOP) extends healthcare services by fostering strong partnerships with longstanding community organizations and addressing the unique needs of micro communities within the Denver Metro area.
Click here to request the HOP for your site. Please be aware that there are days that are already booked with regular standing HOP visits. Not every day/time that your site may be interested in will be available. Requests for new sites or special events must be submitted at least 3 weeks prior to the requested date of service since provider staffing needs to be arranged. To ensure the most efficient use of our resources, our benchmark is typically that a provider should see at least 10 patients across a full day.
The John Parvensky Stout Street Recuperative Care Center provides safe, dignified, and quality spaces for people experiencing homelessness to heal and stabilize from medical issues. The center features 75 medical respite beds targeted to people experiencing homelessness with acute medical conditions. Recuperative care will be provided for approximately 14 days, depending on the needs of the client. All clients must be referred to recuperative care by a healthcare provider or healthcare-affiliated community partner.
