IMPORTANT UPDATE: Week of Monday, November 18

  • The PATH program’s intake form will be closed starting Monday, November 18.
  • There will be no intake appointments on Wednesday, November 27.
  • The form will reopen on Monday, November 25. People can then submit their information for the next intake day on Wednesday, December 4.

All CCH healthcare sites and offices will be closed on Thursday, November 28 and Friday, November 29 for the Thanksgiving holiday. For emergency shelter during severe weather, please [click here].

 

Case management appointments for single adults and adult couples has moved to an online process. Watch the video and complete the form below to sign up for a case management appointment.

If you need help applying for housing or understanding housing options, please review the drop-downs below for specific information for families or single adults/couples.  Note: These programs do not provide shelter, motel/hotel vouchers, or any immediate housing.

We randomly select 20 individuals per week for an appointment. If you are not selected this week, please sign up the next week.
